COMMUNITY PROGRAM MONITOR
GENERAL STATEMENT OF DUTIES:
Under general supervision, coordinates client referrals for evaluation and/or treatment and tracks client progress and follow-through. Performs related work as required.
DISTINGUISHING FEATURES OF THE CLASS:
The Community Program Coordinator is responsible for coordinating client referrals to a specific treatment group or community agency for evaluation and/or treatment services. The position also tracks and reports client progress and serves as the primary community link among the justice system and related community agencies.
EXAMPLES OF WORK: (illustrative only)
Establish and maintain necessary communication and formal agreements for client referrals from justice system components and effect and accountable operation of the program.
Establish and maintain necessary linkages between the program, appropriate community agencies, and the justice system to ensure the appropriate use of the program.
Ensure program integrity and organization capability to carry out the program mission and meet expectations of the justice and treatment systems.
Provide information to agency administrators and others for managing and developing program services, determining operational effectiveness, providing appropriate information to funding sources, and meeting public information needs.
Assist in policy and procedure development, to ensure the clients are promptly screened and entered into the program.
Ensure effective and efficient tracking and case management of clients’ progress through the system.
DESIRED KNOWLEDGE, SKILLS, AND ABILITIES:
Knowledge of program-related treatment modalities and services.
Good general knowledge of underlying principles and practices of Department of Correctional Services operations and/or the ability to readily acquire such knowledge.
Ability to communicate effectively both orally and in writing.
Ability to deal tactfully and courteously with clients, evaluation/treatment providers, employees, and the general public.
Ability to keep records and make reports.
Ability to make presentations to lay and professional groups.
Ability to plan and organize.

REQUIRED EXPERIENCE AND TRAINING:
Associate of Arts degree from an accredited college or university and two years in human services, criminal justice, or corrections experience; or
An equivalent combination of education and experience to equal four years.
ESSENTIAL FUNCTIONS:
Ability to travel to various work sites, possession of or ability to obtain a valid driver’s license, and ability to be insured under the district’s auto insurance coverage.
Ability to communicate with the offender population, criminal justice and related agency staff, and other involved parties in individual and group settings.
Ability to assess offender characteristics and circumstances and organize internal as well as external resources to address offender risk management and treatment plans.
Ability to document observations, assessments and other related information in offender case files within specific time frames.
Ability to comprehend critical information and complex issues to make decisions within established policy guidelines, time frames and legal parameters.
Ability to travel to various work sites and participate in various work activities such as conducting available chemical substance use tests, court/administrative hearings and other related community functions, as required by the duties of the position.
Ability to utilize available technology in the performance of job duties.
